I am currently taking on new patients by referral but please expect to wait a few months for an appointment.  Please email me directly ( if you have questions or call the office to schedule.

Please email or bring the three completed forms (Patient Intake, Patient Information, and Privacy Policy) to your first appointment.  You can find them here:  Forms.  Alternatively, you are welcome to come twenty minutes early to fill out this initial, mandatory paperwork. This will allow us to use your time during the appointment more effectively. Thank you.


Payment Policy

Full payment of supplements and co-payments are expected at the time of service. If you have not confirmed coverage by your health care provider or you have a deductible to meet, I will expect full payment at the time of service. If and when your insurance company should pay me, I will then refund you.


Cancellation Policy

Last minute cancellations of scheduled appointments are both difficult to fill, and costly. Therefore, I ask that cancellations be made at least 48 hours previous to your appointment- not including weekends. Appointments missed or cancelled in less than 48 hours will incur a $75 charge. Exceptions to this policy may be made for emergency situations on a case-by-case basis.

The cost of these appointments are $300/hr and must be paid in full up front. The cancellation policy listed above applies to consultations as well.  Please fill out the required paperwork in the Forms link under Resources and upload them to your Elation passport, which will be set up when you schedule.
